The Gloucester County Caregiver Support Program responds to the needs of CAREGIVERS, providing them with support and assistance in accessing services to help maintain individual caregiving needs.

C.A.R.E.S. Program

The program offers access to services that help families to ease the emotional physical and financial strain of caregivers by providing:

  • Short-term counseling:     
  • In-Home support services (to act as short-term respite for the caregivers)
 
"Circle of Caring Support Group"

Care Connection Program
The Care Connection Program offers an extensive care management program for caregivers providing care for a loved one with Alzheimer's or Dementia related illnesses. Established in 2003 by the Gloucester County Board of Chosen Freeholders, Care Connection is designed to service up to 30 families per year. Services include: Individualized care plans, on-going assessments, monthly reviews, special disease education, behavioral interventions, counseling, emotional support, and if appropriate, an allotment of in-home services and Adult Day Care to act as a period of "respite" for the caregiver.
Placement Services
Established in 2002 by the Gloucester County Board of Chosen Freeholders, the Placement Services program provides hand-over-hand assistance to guide families through the entire selection and application process of choosing a long-term placement facility, serving approximately 150 families per year. A public health nurse evaluates the needs of loved ones and their family members, informs them of available options , and (as necessary) accompanies them on tours of selected facilities
